Health questions may be submitted to the Nielsen Clinic, along with individual contact information in the Ask Our Doctors area of the site. This form also offers Ask Our Doctors users a future contact option.  

The "Nielsen Clinic Survey": 
During any number of site promotions, visitors may be encouraged to fill out an online survey for Nielsen Homeopathic Medical Clinic. This survey asks for contact information and user profile information like sex, age, education, internet connection, whether a family member has an ailment/disease, area of health interest and occupation. These questions help us define the needs of our users. We also ask if the visitor would allow the Nielsen Homeopathic & Integrative Clinic to contact them in the future via email.  

We may share information from surveys with third parties who perform data management services for our site. Those third parties have agreed to keep all data from surveys confidential. Also, we may share information from surveys in an aggregated, de-identified form with third parties with whom we have a business relationship.

IP addresses
We collect and log the IP address of all visitors to NielsenClinic.com. An IP address is a number automatically assigned to your computer whenever you access the Internet. IP addresses allow computers and servers to recognize and communicate with one another. We collect IP address information so that we can properly administer our system and gather aggregate information about how our site is being used, including the pages visitors are viewing.
